The following paperwork is required to set up a new patient:

1. Patient Demographics
2. Insurance Information (if applicable)
3. Signed Prescription
4. Diagnostic/Original Sleep Study
5. Titration Sleep Study (if applicable)

If only supplies are needed and not a new machine, the sleep studies are not required.

The patient will be contacted within 24 hours to schedule an appointment.
